Booklet, 4" x 5-1/2" high, pp [vi], 15.First leaf, pp. [i], a printed form perforated for removal that was to be filled in by employee and returned to office as acknowledgement of receipt of the rule book.
In 1964 company was still called Railroad; in 1968 it became Railway. Form number is the same as the one used in 1964 as the Railroad. Revision and thus earliest issue date was July 1971.
Note: the spelling of "employee" with one 'e' was consistently used by both the Delaware, Lackawanna & Western R.R., the Erie, the merged railroad - the Erie Lackawanna Railroad and as the Railway Company.
