Printed paper form from book, 3-3/4" high x 6-3/8" wide. Serial number. Stapled as group. Reverse blank except for signature of named recipient.

All were for purchases to the amount of three dollars.

Holographic entries, all signed by the Overseer of the Poor, Harry L. Barck, Jr. Issued per order of a named member of the Committee on Alms.
