PDF on file.
Stiff printed wrappers, side-stitched. pp 92 ([1-7], 8-89, [90-92]), plus 2 inserted leaves not in pagination (with information that was received too late for regular inclusion and are titled as insert page 1 of 2 and 2 of 2).
All text is mimeographed on one side of a leaf except for title page leaf, 10-3/4" high x 8-3/8" wide.
The general arrangement of information is an alphabetical list of church denominations with local churches then listed alphabetically under them (not in town order). The listing section for each denomination usually starts with the address of the regional or national headquarters of that particuliar denomination. This is followed by an entry for each church which includes its full name and address along with a clergy or laity and their local address and phone number.
Table of contents (pages [3-6]) lists church denominations alphabetically followed by Volunteers of America and unaffiliated churches. Following the table of contents is a table (page ) with abbreviations and telephone exchanges. The listings follow it. Three indexes start on page 62: church name, place, clergy and laity. (It is presumed the 2 insert pages have information not indexed.) Pages [90-92] is a list of other Historical Records Survey publications.
